HELP Interdisciplinary Follow-Up Care. Our experience with both direct and remote care services is that patients will occasionally need to reconnect to our treatment staff as a group for individualization of their continued clinical path. Sometimes these visits are prompted by the patient setting new goals for higher levels of independence and productivity, other times the visits are necessary to help them overcome a barrier they have found while pursuing their clinical pathway at home.

These visits are two hours in length and include individualized medical, psychological and rehabilitative services. A specific goal is established for these visit prior to scheduling, the conclusions are integrated into the remote care goal setting and activity monitoring.
